Questionnaire planning

I have decided to use general questions in my questionnaire. Also, I think uploading it on my blog instead of emailing it will collect more responses.

    What else do you need to consider when producing a questionnaire? How many questions will you ask? What TYPES of questions will you use? Are all questionnaires the same?
